本研究旨在探究COVID-19疫情期間,護理師於照顧確診者之心理壓力,以及專業承諾的變化經驗。本研究採質性研究中現象學的觀點,以立意取樣的方式,邀請五位具「護理師專業證書」、「臨床工作滿兩年以上」及於「加護病房照護確診者」之女性受訪者,進行半結構式訪談,並進行研究資料的歸納與分析,研究結果說明如下:
一、護理師照護確診者有四項心理壓力主題,分別是「環境引起的束縛:照護環境與設備的限制」、「建立新團隊的為難:重組團隊合作所面臨的挑戰」、「照護面臨的衝擊與孤獨:獨立照護的總總限制」、「維持日常生活的困難:護理師與家人分開,同時改變正常生活型態」。
二、護理師照護確診者專業承諾變化歷程,共可分成四期來看:「疫情前:培訓終成一位重症專業照護者」、「疫情早期:始終敬業並累積新照護和團隊經驗」、「疫情中期:藉疫情衝擊重整自我護理角色與價值」、「疫情共存:暫定專業發展並做階段性承諾」。
研究者依據本研究過程與結果,提出研究限制,並提供護理師、諮商心理師、制度與法規推動,以及未來相關研究之建議。
The purpose of this study is to explore the psychological stress and professional commitment changes experienced by nurses in caring for confirmed patients during the COVID-19 epidemic. This study adopts a qualitative research phenomenological approach and utilizes purposive sampling. Five female participants, who possess a " Registered professional nurse license," have worked in clinical settings for over two years, and have experience in caring for confirmed cases in "intensive care units or dedicated COVID-19 wards," are invited as research interviewees. The study conducts semi-structured interviews and uses thematic analysis to analyze the research data. The results of the study are presented below:
1.The four main psychological stress themes experienced by nurses in caring for confirmed patients are as follows: "Restrictions imposed by the environment: limitations of the care environment and equipment," "Challenges in establishing a new team: Difficulties faced in reorganizing teamwork," "Impact and loneliness in caregiving: Various limitations in providing independent care," and "Difficulties in maintaining everyday life: Separation from family and changes in normal lifestyle for nurses,"
2.This process of change can be divided into four stages: "Pre-pandemic: Training culminates in becoming an expert in critical care," "Early pandemic: Continuously dedicated and accumulating new caregiving and teamwork experiences," "Mid-pandemic: Rebuilding self-nursing roles and values in response to the impact of the pandemic," and "Coexistence with the pandemic: Provisional professional development and making phased commitments."
Based on the research process and findings, the researcher identifies the limitations of the study and provides recommendations for nurses, counseling psychologists, institutional and regulatory amendments, as
