    題名: 以多元文化園藝療育降低失智症外籍看護 照顧負荷之研究
    Research on the Use of Multicultural Horticultural Therapy and Education to Reduce the Perceived Workload Careing Burden of Foreign Nursing for Dementia

    摘要: 本研究針對照顧失智症者之外籍看護工,提供多元文化取向的園藝療育團體。運用園藝療育團體之介入模式,一方面期待透過園藝療育的表達性藝術非語言介入,提供外籍看護在舒壓與降低照顧負荷感;另一方面,本研究也期望在個別晤談過程中,參照晤談過程中,分析了解失智症外籍看護從園藝療育獲得的經驗為何,進而探討失智症外籍看護在園藝療育過程的人際支持系統、失智症外籍看護的照顧負荷感。研究方法為質性研究,邀請四對家庭,有超過一年以上的照顧失智症者經驗之外籍看護,進行一週一次,每次活動為 90 分鐘,共五週。在研究結果分析,因其中一組無法繼續參與,因此最後結果呈現上只有三組。研究設計採「單組前測 ─ 後測設計」,以照顧者自我壓力測驗表,以及訪談法探究多元文化園藝療育課程對失智症外籍看護工的照護負荷感方面之立即和持續效益成效。研究結果說明如下:
    一、 園藝療育團體過程失智症外籍看護個案呈現的「照顧負荷困難」,當被照顧者有身體上的不適時,照顧者感受之負荷越重,行為問題經常困擾了外籍看護,影響外籍看護的照顧負荷增加。
    二、 園藝療育團體過程失智症外籍看護個案產出作品的無顯著文化差異。
    三、 透過多園藝療育團體對失智症外籍看護個案在照顧負荷的助益,增進成員的互動交流,在這樣的凝聚力下,使人與人之間距離拉近,具有提升正向情緒之效益並能達到心情放鬆。
    四、 多元文化園藝療育團體帶入生活,研究後持續追蹤結果,每一位參與者仍持續在家中進行園藝相關活動,共同觀賞植物,而此過程正是一段彼此相互陪伴的時刻,在這樣的狀態下,兩人的身心狀態也可以因此獲得療癒。
    This study aimed at providing a multicultural horticultural therapy and education for foreign caregivers caring for people with dementia. Using the intervention mode of horticultural therapy and education groups, on the one hand, it is expected to provide foreign nursing care in stress relief and reduce the sense of care load through the expressive art non-verbal intervention of horticultural therapy; On the other hand, this study also aims to analyze and understand the experience gained by foreign caregivers with dementia from horticultural therapy and education during individual interviews, and then explore the interpersonal support system of foreign caregivers with dementia in the process of horticultural therapy and the sense of care load of foreign caregivers with dementia. The research method was a qualitative study, inviting four pairs of families with more than one year of experience in caring for dementia patients to conduct a weekly activity of 90 minutes for a total of five weeks. In the analysis of the study results, because one of the groups could not continue to participate, the final results presented only three groups.
    The study design adopted a "single-group pre-test-post-test design" to explore the immediate and sustained effectiveness of multicultural horticultural gardening therapy courses on the perception of care load among foreign caregivers with dementia using a caregiver self-stress test form and an interview method.
    The results of the study are described below:
    1. The "care load difficulty" presented by foreign caregivers with dementia in the process of horticultural therapy groups, when the caregiver is physically unwell, the heavier the load felt by the caregiver, the behavioral problems often plague the foreign caregiver, affecting the care load of the foreign caregiver.
    2. There is no significant cultural difference in the output of foreign nursing cases of dementia in the process of horticultural therapy groups.
    3. Through multi-horticultural therapy and education groups, the care load of foreign care cases with dementia is enhanced, and the interaction between members is enhanced, so that the distance between people is narrowed, which has the effect of enhancing positive emotions and achieving relaxation.
    4. The multicultural horticultural therapy and education group brings the results to life, and the results are continuously tracked after the study, and each participant continues to carry out gardening-related activities at home and view plants together, and this process is a time of mutual companionship, in which the physical and mental state of the two can also be healed.
