    Title: 玻尿酸癒創敷料之開發及其在真皮缺損及糖尿病潰瘍之應用

    Abstract: 玻尿酸(hyaluronic acid)是人體中重要的細胞外基質(extracellular matrix, ECM),其結構為重複N-acetylglucosamine及D-glucuronic序列之糖.胺基聚醣(glycosaminoglycan, GAG)。由於玻尿酸具有獨特的水合(hygroscopic)、黏彈(viscoelastic)、非抗原性(non-antigenic)、生物降解性(biodegradable)及生物相容性(biocompatible)等特性而廣泛應用於生物醫學領域。在創傷癒合過程中,玻尿酸敷料不僅可提供濕潤的環境促進創傷癒合外,逐漸降解並釋放之低分子量玻尿酸分子更能扮演促進細胞增生、細胞遷移、血管新生及減少瘢痕組織等重要生理功能。許多研究指出,創傷若能與較低低分子量之玻尿酸接觸將可促進創傷癒合;但未經交聯反應之玻尿酸敷料卻無法提供適宜之生物安定性,或者是常造成過度分解而與傷口沾黏之現象。為了瞭解不同製備方法對玻尿酸分子於創傷中的釋放行為(release behavior)及對創傷癒合之影響,本研究擬以玻尿酸溶液與環氧化合物 (poly (ethylene di-glycidyl) ether, EX-810)分別製成完全交聯、未交聯之海綿狀玻尿酸敷料及上述兩種基材重疊組成之雙層敷料。藉觀察其顯微結構(microstructure)及紅外線光譜(FTIR spectrum)之瞭解材料特性之差異;以玻尿酸分解酵素抵抗性(resistance to hyaluronidase)與自由基抵抗性(resistance to free radical)模擬其在生理環境下降解之情形。並以含纖維母細胞之膠原蛋白膠體(fibroblast-populated collagen gel)接觸模擬玻尿酸敷料與真皮層接觸時是否會產生細胞毒性物質。動物試驗除以SD 大鼠真皮層缺損模式(full-thickness defect model)之外,更進一步探討其在糖尿病小鼠(NOD/LtJ)之壓力性潰瘍(pressure ulcer)癒合過程中所扮演之角色。
