    Abstract: 囊性水瘤是一種發生於兩歲前孩童的淋巴管異常,主要侵犯頭頸部,成人甚為罕見。本篇報告一例53歲女性因頸部腫塊,由開業醫以囊性甲狀腺腫轉介來。超音波檢查發現位於甲狀腺及頸動脈外側之囊性腫瘤,電腦斷層攝影此囊腫位於胸鎖乳突肌後側方並壓迫到內頸靜脈。手術時發現在胸鎖乳突肌下面有一含黃色液體之囊腫,壓迫並粘連到內頸靜脈,經小心剝離,將腫瘤完全切除。病理報告證實為囊性水瘤。經追蹤二年沒有復發。囊性水瘤在成人頸部囊腫,需要列入鑑別診斷。完全切除而不犧性重要構造。為治療之選擇。
    Cystic hygroma is a congenital lymphatic malformation involving mainly the head and neck region in children before age 2 years old. It is seen rarely in adults. We report a 53 year-old female referred by a local physician as cystic goiter. Sonography showed a cystic mass lateral to thyroid and carotid artery. Computed tomography depicted a low density lesion posterior lateral to sternocleidomastoid muscle with compression of internal jugular vein. Complete excision was performed without recurrence. Cystic hygroma must be considered in the differential diagnosis of a cystic mass of the neck in adults. Complete excision without sacrifice of vital structure is the treatment of choice.
